Job summary

CenterWell Home Health is seeking a Home Health Physical Therapist to deliver in-home physical therapy, helping patients regain strength and independence. You will report to the Clinical Manager and collaborate with a multidisciplinary team to plan and implement personalized rehabilitation plans.

Responsibilities include assessing progress, documenting outcomes, guiding PTAs and HHAs, and communicating with physicians, patients, and families.

Qualifications

• Current and unrestricted Physical Therapy license.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.

Responsibilities

• Plan and administer prescribed physical therapy in patients' homes to improve function.
• Assess and document patients' physical strengths to guide treatment goals.
• Develop and implement rehabilitation plans aligned with physician orders and patient goals.
• Provide clear care instructions to PTAs and Home Health Aides and supervise their work.
• Document observations, interventions, and progress promptly.
• Report patient progress to the physician, Clinical Manager, and family, including discharge summaries.
