Job summary

Assured Home Health, a part of LHC Group and within the Optum family, seeks an Occupational Therapist to deliver home health services in Washington. You will assess functional status, ADLs, and adaptive equipment, collaborating with physicians and other health-care team members to implement the plan of care.

The role requires WA OT licensure, a valid driver’s license, and CPR certification (preferred or within 90 days).

Qualifications

• Current Washington state Occupational Therapy licensure.
• Valid driver's license and access to a vehicle or reliable transportation.
• CPR certification preferred or ability to obtain within 90 days of hire.

Responsibilities

• Assess and evaluate the patient's functional status, activities of daily living, and need for adaptive equipment.
• Perform home assessments and coordinate with the physician to develop and update the plan of care.
• Observe, document, and report patient responses to treatment to the nurse supervisor and physician.
• Deliver occupational therapy services within state scope of practice and CMS-compliant documentation.
