Job summary

ExcelaCare Home Health in Big Spring, TX seeks an Occupational Therapist (PRN) to provide skilled therapy in patients’ homes. You will assess, plan, implement, and monitor treatment programs to restore function and reduce pain, working under physician guidance.

You will educate patients and families, document progress, and collaborate with an interdisciplinary team to ensure safe, effective care consistent with agency policies and standards.

Qualifications

• Education: Graduate of an accredited Occupational Therapy program.
• Experience: Two (2) years of experience as an occupational therapist in the last five (5) years.
• Training: Appropriate competency assessments completed.
• License / Certification: Current licensure; NBCOT; CPR certification; valid driver’s license and auto liability insurance.

Responsibilities

• Assess the patient’s functional needs and set goals with physician input.
• Reassess and adjust care plans to optimize outcomes.
• Provide direct patient care and teach home exercise programs.
• Document evaluations and treatment goals in a timely manner.
• Coordinate with other disciplines to ensure quality care.
• Participate in on-call rotation as required.
