Job summary

ABL Health Care is seeking a compassionate Occupational Therapist (OT) for Home Health in the San Rafael area. The role is part-time per diem, serving San Rafael, Novato, Mill Valley, and Corte Madera with field responsibilities in patients’ homes.

The OT will evaluate patients, develop and implement care plans, and monitor progress, collaborating with Clinical Director and staff to ensure high-quality care. California licensure and driving within a 15–25 mile radius are required.

Qualifications

• Current OT license to practice in California – Required.
• Experience in Home Health or Hospice care – 1 Year Required.
• Current California driver’s license.
• Able to travel within a 15–25 mile radius on a regular basis.

Responsibilities

• Provide, coordinate and oversee skilled intermittent care to patients in the home.
• Regularly evaluate and re-evaluate patient status.
• Administer treatments, monitor patient conditions, and report signs/symptoms to relevant staff.
