Job summary

Commonwealth Home Health seeks an Occupational Therapist Assistant to deliver home-based therapy and restore patients' physical abilities. This full-time role supports the supervising OT and a collaborative care team.

Responsibilities include applying evidence-based treatments, documenting progress, and educating patients and families. A valid state license, transportation flexibility, and CPR certification are required or preferred.

Qualifications

• Current Occupational Therapy Assistant licensure in the state of practice.
• Current driver's license and access to dependable transportation or vehicle insurance.
• CPR certification or ability to obtain within 90 days of hire.

Responsibilities

• Follow state laws and supervise treatment plans defined by the supervising OT.
• Instruct and aid patients in evidence-based therapy within scope and state rules.
• Observe, document and report patient responses and changes to care team.
• Educate patient, family, and caregivers in therapy activities and safety.
• Provide care in diverse home settings regardless of patient background.
