LHC-Illinois Home Health Care, part of LHC Group, seeks an Occupational Therapist to deliver in-home therapy, assess daily living activities, fine motor skills, and equipment needs, and collaborate with physicians and nurses.

You'll evaluate functional status, document CMS-related data, and contribute to care plans. A current OT license in the state and CPR certification are required; a dependable vehicle is preferred.

Formación

• Current Occupational Therapy licensure in state of practice.
• Current CPR certification.
• Current driver's license and vehicle insurance or access to dependable transportation.
• Ability to work in diverse home environments.

Responsabilidades

• Assess and evaluate patient care needs related to functional status and activities of daily living.
• Collaborate with physicians and other health care team members on therapy plan of care.
• Record patient responses and changes, and document CMS-related information.
• Conduct functional ability assessments using standard tests.
