Job summary

LHC-Illinois Home Health Care, part of the Optum family, is seeking an Occupational Therapist to evaluate and treat patients in their homes across Illinois. You will assess functional status, daily living activities, and home safety, coordinating with care teams to deliver personalized therapy plans.

Required qualifications include state OT licensure, CPR certification, a valid driver's license with reliable transportation, and strong communication skills. Competitive pay and benefits offered.

Qualifications

• Current Occupational Therapy licensure in state of practice.
• Current CPR certification.
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.
• Strong communication, writing, and organizational skills.

Responsibilities

• Provides service within the scope of practice as defined by state laws governing Occupational Therapy and CMS plan of care.
• Evaluates patient’s functional status and collaborates with physician on therapy plan of care.
• Observes and reports patient response to treatment to nurse supervisor and physician.
• Conducts patient assessments evaluating functional ability.
