Job summary

CenterWell Home Health is seeking a Home Health Registered Nurse to deliver one-on-one skilled nursing care in patients’ homes, including medication administration, wound care and comprehensive assessments.

Reporting to the Clinical Manager, you will develop individualized care plans with physicians, monitor progress, educate families on disease management, and coordinate with the care team to support recovery and independence.

Qualifications

• Med-surg, ICU, ER, or acute care experience.
• Current and unrestricted RN license.
• CPR certification.
• Experience collaborating with a healthcare team.
• Valid driver's license and vehicle insurance.

Responsibilities

• Provide skilled nursing care to patients in their homes, including medications, wound care, and assessments.
• Develop and implement individualized care plans with physicians and clinicians.
• Monitor patients’ conditions and report changes to the physician and Clinical Manager.
• Educate patients and families on disease management, medications, and treatment options.
• Maintain accurate patient care records and coordinate with the care team.
• Report patient progress to the physician and Clinical Manager.
• Oversee Home Health Aides and ensure documentation is completed correctly.
