Humana Inc. is seeking a Home Health Registered Nurse to provide personalized, one-on-one care in patients' homes, reporting to the Clinical Manager. You’ll manage care plans with physicians and clinicians to support recovery and independence.

The role requires med-surg/ICU/ER exposure, at least one year of nursing experience, current RN licensure, and CPR certification. A valid driver's license and reliable transportation are required.

Formación

• Med surg, ICU, ER, or acute experience.
• One year nursing experience.
• Current and unrestricted RN licensure.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• Valid driver’s license, auto insurance and reliable transportation.
• Preferred: Home Health experience.

Responsabilidades

• Provide skilled nursing care to patients in their homes, including administering medications, wound care, and patient assessments.
• Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals.
• Monitor patients' conditions and report changes.
• Educate patients and their families on disease management, medication, and treatment options.
• Maintain accurate records of patient care and coordinate with other healthcare professionals.
• Report patient care and condition progress to patient’s physician and Clinical Manager.
• Oversee the work of Home Health Aides and monitor documentation completion.

Conocimientos

Med surg / ICU / ER experience

One year nursing experience

RN licensure (current & unrestricted)

CPR certification
