Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Ability to travel to patient homes as needed.

Responsibilities

• Provides high quality clinical services within the scope of practice per plan of care and infection control procedures.
• Documents patient visits thoroughly in the electronic medical record.
• Participates in care coordination, reports patient needs and condition changes, and communicates with patient, family, physician, and care team.
• Functions effectively in diverse home environments and with diverse patient populations.
