Qualifications

• Licensed Practical Nurse (LPN) in Colorado.
• 3 years nursing experience; home health or medical/surgical preferred.
• CPR certification required and valid license.

Responsibilities

• Provide direct patient care per the state Nurse Practice Act.
• Implement care plans initiated by the registered nurse.
• Document accurately and timely according to the plan of care.
• Educate patient and family/caregiver on self-care and diagnosis as needed.
• Coordinate home health services with other disciplines and report needs to RN or supervisor.
• Use equipment and supplies adhering to aseptic technique.
