Human Touch Home Health Care in Colorado is seeking a PRN Licensed Practical Nurse to provide direct patient care under RN supervision. Responsibilities include implementing the plan of care, documenting procedures, and educating patients and families on self-care.

You will coordinate with the team to ensure quality home health services and maintain aseptic practices. Requirements include graduation from an accredited LPN/VN program, Colorado LPN license, and CPR certification.

Qualifications

• Graduate of an accredited practical nurse or vocational nursing program.
• Licensed as an LPN or LVN in Colorado.
• CPR certification maintained.

Responsibilities

• Provide direct patient care under RN supervision.
• Follow plan of care and document timely and accurately.
• Educate patients and families on self-care per diagnosis and plan of care.
• Coordinate home health services and report needs to RN or supervisor.
• Maintain aseptic technique with equipment and supplies.

Responsibilities including following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes.

Responsibilities:

• Provides direct patient care as defined in the state Nurse Practice Act.
• Implements plan of care initiated by the registered nurse.
• Provides accurate and timely documentation consistent with the plan of care.
• Assesses and provides patient and family/caregiver education regarding self-care techniques and information pertinent to diagnosis and plan of care.
• Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietitian) to the registered nurse and/or clinical supervisor.
• Uses and/or prepares equipment and supplies effectively and efficiently adhering to aseptic technique.
• Participates in personal and professional growth and development.
• Performs other duties as assigned by the registered nurse.

Qualifications:

• Graduate of an accredited practical nurse or vocational nursing program.
• Has three (3) years nursing experience. Community/home health or medical/surgical experience is preferred.
• Currently licensed as a licensed practical nurse or licensed vocational nurse in the state.
• Possess and maintain valid CPR certification.
• Complies with accepted professional standards and practice.
• Demonstrates good verbal and written communication and organizational skills.
• Licensed driver with automobile that is in good working order and insured in accordance with the organization requirements.

Job Type: Full-time

Pay: From $20.00-50.00 per visit
