CarePartners Health Services in Asheville, NC, is seeking a Licensed Practical Nurse (LPN/LVN) to join our Home Health Team. You will provide compassionate, in-home nursing care in collaboration with RNs and the care team to promote independence and safe care.

Responsibilities include wound care, medication administration, patient education, and clear documentation. Must have an active LPN/LVN license and a valid NC driver's license; training in basic life support required within 15 days of

Qualifications

• Basic Cardiac Life Support must be obtained within 15 days of employment start date.
• Licensed Practical or Vocational Nurse (LPN/LVN) license active.
• Associate Degree or Vocational School graduation.
• DL - Driver License in good standing.

Responsibilities

• Providing nursing care in the home, including wound care, medication administration, and support during procedures
• Documenting assessments, interventions, and outcomes clearly and thoroughly to support safe, continuous care
• Monitoring patient status and alerting the care team of any changes to ensure timely, coordinated responses
• Educating patients and families to support medication adherence and health understanding
• Collaborating with RNs, providers, therapists, and care coordinators to deliver high-quality, personalized home care
