Job summary

CarePartners Health Services, a member of Mission Health and operating division of HCA Healthcare, delivers compassionate, at-home care across western North Carolina. We are seeking an LPN/LVN to join the Home Health team in Asheville, providing individualized, family-centered care in patients' homes.

You will work closely with RNs and the broader care team to manage medical needs, promote independence, and support safe, effective home-based care during visits.

Qualifications

• BLS certification must be obtained within 15 days of employment start date.
• Driver license required.
• Licensed Practical or Vocational Nurse (LPN/LVN) license appropriate for state.
• Associate degree or vocational school graduate credential.

Responsibilities

• Provide nursing care in the home, including wound care, medication administration, and procedures.
• Document assessments, interventions, and outcomes clearly for safe, continuous care.
• Monitor patient status and alert care team to changes for timely response.
• Educate patients and families to support medication adherence and independence at home.
• Collaborate with RNs, providers, therapists, and care coordinators to deliver high-quality home care.
