Job summary
The Pennant Group (Beacon Home Health) is seeking a Licensed Practical Nurse (LPN) to join the Cookeville, TN home health team. The LPN will provide skilled nursing care in patients' homes, following the plan of care and coordinating with the health-care team.
Responsibilities include patient assessment, family education, and documentation. Must maintain CPR certification, have a valid driver's license and reliable transportation for travel between patient residences daily.
Qualifications
- LPN licensure in the state of practice.
- CPR certification required.
- Valid driver's license and reliable transportation.
- Ability to commute daily to multiple patient residences within the area.
Responsibilities
- Provide skilled nursing care in patients' homes.
- Follow the plan of care and coordinate with health-care team.
- Instruct family on patient care and safety.
- Document patient status and report changes to supervising nurse/physician.
- Collaborate with physicians and RNs; do not alter care plan.
