Job summary

The Pennant Group is seeking a Licensed Practical Nurse (LPN) to join Beacon Home Health serving Wilson County, Mt. Juliet, and surrounding communities. The role covers patient homes on the road, providing skilled nursing care according to the plan of care.

Responsibilities include patient assessment, family instruction, and coordination with physicians and RNs. Applicants must have an active LPN license, 1 year of clinical LPN experience, CPR certification, and reliable transportation.

Qualifications

• One year of clinical LPN experience.
• Current LPN license in the state where services are rendered.
• Current CPR certification, driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
• Must be able to reliably commute to multiple patient residences daily within the geographic area assigned.

Responsibilities

• Provide skilled care in the patient’s home within the scope of practice and plan of care.
• Instruct family in patient care and maintain a healthy home environment.
• Observe, record, and report the patient’s response to treatment and any changes to supervising staff.
• Collaborate with physicians and RNs and document care promptly.
