VitalCaring Group in Port Orange, FL seeks a Licensed Practical Nurse (LPN/LVN) for field-based home health care. You will provide direct patient care in patients' homes under RN supervision, helping implement care plans and support recovery and independence.

This role emphasizes one-on-one patient relationships, flexible scheduling, and collaboration with an interdisciplinary team. A valid FL license, CPR, and driving ability are required; home health experience is a plus.

Formación

• Licensed LPN/LVN in the state of practice.
• Graduate of an accredited practical/vocational nursing program.
• Current CPR certification.
• Minimum one (1) year nursing experience preferred (Home Health, Med-Surg, or Acute Care a plus)

Responsabilidades

• Provide direct patient care under the supervision of an RN.
• Deliver skilled nursing services according to the plan of care.
• Monitor patient conditions and report changes to the RN and care team.
• Educate patients and caregivers on self-care, treatments, and disease management.
• Maintain infection control and proper use of equipment and supplies.
• Document patient care accurately and timely within the EMR.
• Communicate effectively with physicians, nurses, and interdisciplinary team members.
• Participate in case conferences, team meetings, and ongoing training.
• Support coordinated care by maintaining consistent communication across the care team.
