Job summary

Qualifications

• Graduation from an accredited School of Practical Nursing.
• Active LPN license in the state.
• One or more years of experience in community/home health setting preferred.
• Criminal background check required.
• Current CPR certification; online certification not accepted.
• Good communication skills in English (written and verbal) required.
• Bilingual (English/Spanish) strongly preferred.

Responsibilities

• Provide comprehensive skilled care in patients' homes to help them live independently.
• Educate patients and families to promote well‑being and adherence to care plans.
• Collaborate with care teams to ensure updates are reflected in care plans.
