Job summary

Mid Michigan Home Health & Hospice is seeking a compassionate Home Care & Hospice Registered Nurse (RN) to serve Lansing, MI and surrounding communities. You will provide in-home skilled nursing, develop and evaluate care plans, manage medications, and coordinate care with a multidisciplinary team to support patients and families.

The role offers flexible scheduling in a home-based setting, with opportunities to grow in home health and hospice.

Qualifications

• Current, unrestricted Michigan RN license.
• Home health, hospice, or acute care experience preferred.
• Strong clinical assessment and critical-thinking skills.
• Excellent communication and organizational skills.
• Ability to work independently with a flexible daily schedule.
• Valid driver's license, reliable transportation, and automobile insurance.

Responsibilities

Position Responsibilities

• Provide skilled nursing assessments and direct patient care in the home setting.
• Develop, implement, and evaluate individualized plans of care.
• Perform comprehensive patient assessments and monitor changes in condition.
• Provide medication management, wound care, disease management, and patient education.
• Coordinate care with physicians, therapists, social workers, aides, and other members of the interdisciplinary team.
• Provide compassionate hospice care, including symptom management and end-of-life support.
• Educate patients and caregivers regarding disease processes, medications, safety, and care needs.
• Complete accurate and timely clinical documentation in accordance with agency policies and regulatory requirements.
• Communicate effectively with patients, families, physicians, and the clinical management team.
