Job summary

Westminster Canterbury Richmond is seeking a Home Health & Hospice Aide (PRN) to provide compassionate personal care to skilled patients. You will work under the supervision of a registered nurse or therapist, following the plan of care and documenting progress.

Requirements include Virginia Certified Nursing Assistant status, CPR certification, and two years' community health experience. The role offers paid time off, benefits, and ongoing training opportunities.

Qualifications

• Certified Nurse Aide in Virginia in good standing.
• Two years of nursing assistant experience in community health.
• 75 hours of classroom and supervised practical training.
• Current CPR certification for healthcare provider.

Responsibilities

• Provide care for skilled home health patients per plan of care.
• Assist with bathing, grooming, mobility and transfers.
• Document observations and communicate changes to the RN.
• Assist with feeding and toileting needs; manage meds reminders.
• Support discharge planning and coordinate with care team.
