Job summary

Qualifications

• Current Certified Nursing Assistant (CNA) certification in Texas is required.
• High School diploma or GED is required.
• CPR certification or ability to obtain it on hire is required.
• Valid Texas driver’s license, reliable transportation, and auto insurance required.

Responsibilities

• Provide personal care services (bathing, grooming, toileting, hygiene) as per care plan.
• Assist with mobility, transfers, and ADLs to support independence.
• Monitor patient condition and report changes to supervising nurse.
• Document care activities accurately in EMR or designated systems.
• Travel to patient homes within service area for PRN assignments.
