Job summary

Frontpoint Health in Burleson, TX seeks a PRN Certified Nursing Assistant - Home Health to provide compassionate, patient-centered care in patients' homes. You will assist with daily living activities, monitor vital signs and support mobility, reporting changes to healthcare providers as needed.

The role requires current Texas CNA certification, prior home health or similar experience, reliable transportation, and the ability to work both independently and as part of a team.

Qualifications

• Current certification as a Certified Nursing Assistant (CNA) in the state of Texas.
• Experience working as a Home Health Aide or in a similar role.
• Ability to work independently and as part of a team.
• Excellent communication and interpersonal skills.
• Valid driver's license and reliable transportation.
• Ability to pass a background check and drug screening.

Responsibilities

• Assist patients with activities of daily living, including bathing, dressing, and grooming.
• Provide medication reminders and assist with medication administration as directed by the patient's healthcare provider.
• Assist with mobility and transfers, including the use of assistive devices such as walkers and wheelchairs.
• Monitor and record vital signs, such as blood pressure and temperature.
• Assist with light housekeeping tasks, such as laundry and meal preparation.
• Provide companionship and emotional support to patients and their families.
• Report any changes in the patient's condition to the healthcare provider.
