CenterWell Home Health in Independence, MO, is hiring a Home Health Aide to help patients regain strength, mobility and independence in their homes. You will work under the RN and follow the Aide Plan of Care to deliver rehabilitation-focused care.

The role requires a recent CNA or CHHA program completion, state certification, a valid driver's license, and at least one year of HHA/CNA experience in healthcare settings. Expect a patient-facing position with safety and documentation duties.

Qualifikationen

• CNA or CHHA program completed within 24 months.
• Meet state certification requirements.
• Valid driver's license, auto insurance, reliable transport.
• Good standing on HHA Registry; completed an HHA/CNA course for Medicare agency.
• At least 1 year of HHA/CNA experience in healthcare setting.

Aufgaben

• Provide direct patient care under RN direction and Plan of Care.
• Assist with self-administered medications and observe for correct dosing.
• Take and report temperature, pulse, and BP.
• Help with hygiene including bathing, grooming, oral care, and nails.
• Assist with feeding; monitor intake and position for meals.
• Assist with toileting and ostomy care as prescribed.
• Support mobility and transfers per Plan of Care.
• Document care and communications accurately and timely.
• Maintain infection-control practices and a safe environment.
• Help with light household tasks if needed and MD order.
