Job summary

CenterWell Home Health is seeking a Home Health Aide to support patients at home under RN supervision. You will help patients regain strength, mobility and independence through rehabilitation-focused care and daily living support.

The role includes direct patient care, medication assistance, vitals monitoring, hygiene, feeding, mobility, and documentation to ensure high-quality, continuous care in a home setting. A valid HHA/CNA credential and transportation are required.

Qualifications

• Certified Nursing Assistant or Certified Home Health Aide program completed within the last 24 months.
• Must meet applicable state certification requirements.
• Valid driver’s license, auto insurance and reliable transportation.
• Must be in good standing on the HHA Registry and have completed HHA/CNA course to work for a Medicare certified agency.
• Minimum one year experience as a Home Health Aide or CNA in hospital, nursing home, or home health/hospice agency within the last 24 months.

Responsibilities

• Provide direct patient care under the direction of the RN and according to the Aide Plan of Care (POC).
• Assist patients with self-administered medications by helping with bottles, labels, and dosing awareness.
• Take accurate temperature, pulse, and blood pressure and report abnormal results.
• Help patients with personal hygiene (bathing, grooming, skin care, oral care, nails/feet).
• Assist with feeding and communicate basic nutrition, observe intake, and support self-feeding.
• Assist with toileting (bed pan/urinal, catheter bags, ostomy care).
• Provide skills to safely assist patient mobility, exercises, transfers and ambulation per Plan of Care.
• Document care and pertinent information for continuity of care; ensure thoroughly and legibly.
• Practice infection control; provide a clean, safe environment.
• Assist with light household tasks and shopping if needed and allowed.
