Job summary

Wellspring Home Health seeks a qualified Home Health Aide to provide compassionate in-home care to patients in their residences across Ohio. You will follow an established care plan under supervision of an RN or therapist, assisting with daily living activities, monitoring health status, and reporting changes promptly.

The role requires a state-approved HHA training program with certification, at least 75 classroom hours, and a minimum of 1 year direct patient care experience.

Qualifications

• Must have high school diploma or equivalent.
• Completion of a state approved Home Health Aide training program with certification.
• Must have completed at least 75 hours of classroom and supervised practical training.
• Must pass a written competency test prior to working independently.

Responsibilities

• Provide personal care activities as prescribed in the plan of care.
• Assist with hygiene, mobility, and elimination as needed.
• Report observations and changes to the RN or Supervisor promptly.
• Document care provided and patient response in a timely manner.
• Maintain safe environment and comply with confidentiality and HIPAA.
