Job summary

CenterWell Home Health in Ohio is seeking a compassionate Home Health Aide to support patients at home under RN supervision. You will provide direct patient care, assist with medications, monitor vitals, and help with daily activities to improve mobility, independence and quality of life.

Requirements include STNA or CNA/HHA certification within the last 24 months, state certification, valid driver's license and reliable transportation, and at least one year of recent HHA/CNA experience in

Qualifications

Must hold STNA or equivalent certification (within last 24 months)
Must meet state CNA/HHA certification requirements
Valid driver's license, auto insurance, and reliable transportation required
Minimum one year of HHA/CNA experience in healthcare setting
Must be in good standing on HHA Registry (if applicable) and have completed HHA/CNA coursework

Responsibilities

Provide direct patient care under RN direction and according to the Plan of Care
Assist with self-administered medications as instructed and monitor dosing
Take and report vitals (temperature, pulse, BP) and observe for abnormal results
Support personal hygiene and activities of daily living
Assist with feeding and nutrition, ensure safe meals
Assist with toileting and ostomy care as applicable
Support mobility, positioning, transfers and exercises per Plan of Care
Document care provided and communicate changes to team
Maintain infection control practices and safe environment
Assist with light household tasks when needed and allowed
