Job summary

Together We Care Careers is seeking a Home Health Aide to support clients in their homes or facilities under RN supervision. You will perform health-related tasks and provide personal hygiene assistance, helping with daily living activities as needed.

The role emphasizes adherence to the care plan, collaboration with the nursing team, and compassionate, reliable service. Training through NYS DOH basic HHA or equivalent is required to qualify.

Qualifications

• Current NYS DOH approved Basic Training in Home Health Aide, or equivalency/competency program.

Responsibilities

• Carries out the agency’s mission, philosophy, goals and objectives within guidelines of Agency policy and position function.
• Participates in the development and implementation of a realistic goal-oriented plan of care.
• Provides personal care and rehabilitative services and assists in activities of daily living as directed by the Director of Clinical Services or the supervising Registered Nurse.
• Follows care plan instructions as directed by Registered Nurse.
