Job summary

togetherwecancareers is seeking a Home Health Aide to provide health-related tasks and support with personal hygiene, daily living activities, and light housekeeping in clients' homes or facilities under RN supervision.

The role requires completion of NY Department of Health approved HHA training or an equivalency/competency program. You will participate in care planning and execute plan of care while adhering to agency policies and procedures.

Qualifications

• Must have NY Department of Health approved Home Health Aide Basic Training or equivalency/competency program.

Responsibilities

• Performs health related tasks and may assist with personal hygiene, housekeeping and other supportive tasks in home or facility as directed by a registered nurse.
• Carries out the agency’s mission, philosophy, goals and objectives within guidelines of policy and position function.
• Participates in developing and implementing a realistic goal-oriented plan of care.
• Provides personal care and rehabilitative services and assists in activities of daily living as directed by the Director of Clinical Services or supervising RN.
• Follows care plan instructions as directed by RN.
