Job summary

Capital Health Care Network is seeking a Home Health Aide to join our patient-centered care team. Under RN supervision, you will provide essential personal care, observe and report patient status, and help maintain safety and comfort in the patient's home.

Qualifications

• Meets the training requirements in accordance with State and Federal laws.
• At least 18 years of age.
• Ability to read and follow written instructions and document care given.
• Self-directing with the ability to work with little direct supervision.
• Empathy for the needs of the ill, injured, frail and the impaired.
• Prefer that you possess and maintain current CPR certification and First Aid.
• Demonstrates tact, patience and good personal hygiene.
• Licensed driver with automobile that is insured in accordance with Organization requirements and is in good working order.

Responsibilities

• Providing personal care including baths, back rubs, oral hygiene, shampoos, changing bed linen, assisting with dressing and undressing, skin care, toileting assistance, keeping living area clean and orderly, planning and preparing nutritious meals, assisting in feeding if necessary.
• Taking and recording vital signs when ordered.
• Assisting in ambulation and prescribed exercises.
• Performing range of motion and other simple procedures as ordered.
• Assisting patient in self-administration of medication.
• Doing patient’s laundry as appropriate.
• Ensuring safety and proper use of equipment.
• Reporting changes to the assigned nurse.
• Adhering to documentation and care procedures.
