Job summary

VitalCaring is seeking a compassionate Home Health Aide to provide hands-on, in-home care that enhances comfort and quality of life for patients. You will work closely with the clinical team in a home-based setting, delivering respectful, individualized support.

The role requires state and federal HHA certification or training, CPR, and reliable transportation. You will document care in the EMR and engage in ongoing care coordination with nurses and therapists.

Qualifications

• Meets state and federal requirements for Home Health Aide certification or training.
• At least 18 years of age.
• Ability to read, follow care plans, and document services provided.
• Current CPR certification.

Responsibilities

• Provide compassionate, hands-on care that supports patient comfort, safety, and independence.
• Assist patients with daily living activities: bathing, dressing, grooming, toileting.
• Support mobility, transfers, and basic exercises per care plan.
• Maintain a clean, safe home environment.
• Prepare meals and assist with feeding as needed.
• Record vital signs and document services in EMR.
• Observe changes and report to clinical team.
• Participate in team meetings and care coordination.
