Job summary

Melrose Park View Place is seeking a dedicated Home Health Aide to provide compassionate in-home care, helping patients maintain independence and quality of life.

You will assist with daily activities, monitor health, document care, and follow established plans while adhering to safety protocols. Strong communication and reliable transportation are valued. A high school diploma is preferred, and Home Health Aide certification is encouraged.

Qualifications

• High school diploma or equivalent is preferred.
• Previous experience in home health care or as a Home Health Aide is an advantage.
• Compassionate, patient-focused, and reliable.
• Good communication and interpersonal skills.
• Ability to work independently and follow established care plans.
• Current or willingness to obtain Home Health Aide certification preferred.
• Dependable transportation for traveling to patient homes.

Responsibilities

• Assist patients with activities of daily living, including bathing, dressing, grooming, and mobility.
• Support patients with medication reminders and light household tasks.
• Observe and report changes in patients' physical or emotional condition.
• Provide emotional support and companionship to patients.
• Maintain documentation of care provided in accordance with agency guidelines.
• Adhere to safety and infection control protocols
