Job summary

Addus HomeCare in Kingston, TN is seeking a Home Health Aide to provide compassionate, hands-on care in patients' homes. You will assist with daily living activities, prepare meals, transport clients to appointments, and support mobility as outlined in the plan of care.

Responsibilities include documentation and reporting patient changes to the supervising nurse, while maintaining confidentiality and respecting HIPAA guidelines. Reliable transportation and a current driver�s license are required.

Qualifications

One year of hospital or nursing home experience.
Must be 18+ and a high school graduate or equivalent.
Ability to read and follow written instructions and document care given.
Displays empathy for the needs of the ill and frail.
Demonstrates tact, patience, and good personal hygiene.
Possess and maintain CPR certification, if required.
Must have reliable transportation, valid driver�s license and insurance; or ability to take public transportation.

Responsibilities

Providing hands-on personal care including baths, back rubs, oral hygiene, shampoos, changing bed linen, helping with dressing.
Planning and preparing nutritious meals.
Assisting in feeding the patient as needed.
Transport client to doctor�s appointments and essential errands.
Assisting in ambulation and exercise per care plan.
Assisting in self-administration of medications.
Perform other duties as assigned.
