Job summary

Humana Inc. is seeking a Home Health Aide (HHA) to provide direct patient care under RN supervision and according to the Plan of Care. You will assist with medications, vital signs, hygiene, feeding, mobility, and documentation to ensure continuity of care.

The ideal candidate has a high school diploma, CNA/CHHA completion within 24 months, valid state certification, a driver’s license, and at least one year of recent HHA/CNA experience in hospital or home health settings; Scottsdale, AZ.

Qualifications

• High school diploma or equivalent required.
• Completion of Certified Nursing Assistant or Certified Home Health Aide Program within the last 24 months.
• Must meet applicable state certification requirements.

Responsibilities

• Provide direct patient care under RN supervision and according to the Plan of Care.
• Assist with self-administered medications, reading labels and monitoring doses.
• Take and record vital signs (temperature, pulse, BP) and report abnormalities.
• Help with personal hygiene, bathing, grooming, oral care, and assist with feeding.
• Assist with mobility, positioning, transfers, and safe ambulation per care plan.
• Document interactions and care provided for continuity of care and reporting changes.
