Job summary

Adaptive Home Health, LLC is seeking a field-based Home Health Aide in Las Vegas to provide compassionate personal care in patients' homes, support daily activities, and promote independence. You will perform ADLs, assist with transfers, provide medication reminders, monitor status, and document in HCHB, while coordinating with the supervising RN/LVN.

Flexible schedule and mileage reimbursement offered. Competitive hourly pay of $17–$18 based on experience, with benefits and a mission-driven

Qualifications

• Must have Vegas HHA certification.
• At least 1 year of personal care experience in home health, assisted living, or hospital settings.
• CPR/BLS certification required.
• Current Vegas driver’s license with reliable transportation.
• Ability to lift and assist with transfers safely.
• Strong communication and professional demeanor.
• Comfortable working independently in patients’ homes.
• High school diploma or GED.

Responsibilities

• Provide personal care assistance (bathing, dressing, grooming, toileting).
• Assist with safe transfers, ambulation, and mobility.
• Support patients with medication reminders per care plan.
• Monitor and report changes in patient condition and daily living activities.
• Perform light housekeeping related to safety and care.
• Document care in Homecare Homebase (HCHB) accurately.
• Communicate clearly with patients, families and care team for continuity of care.
• Follow infection control practices and maintain patient dignity and privacy.
• Assist with range of motion exercises and restorative care as ordered.
• Coordinate with supervising RN/LVN for care plan updates.
