Job summary

Visiting Nurse Association Community Services seeks a Home Health Aide (HHA)/Direct Care Worker to provide compassionate in-home personal care and support, following each client's plan of care and delivering one-on-one attention in a safe, respectful environment.

You will assist with activities of daily living, monitor health changes, and help maintain a clean, comfortable living space in clients' homes. This role emphasizes empathy and reliability.

Qualifications

• Must be at least 18 years old.
• Completion of a formal certification program or ability to pass a skills/competency test.
• Ability to read, write, and follow a written Plan of Care.
• Good verbal, written, and interpersonal communication.
• Neat appearance and good personal hygiene.
• Dependability, reliability, and sound judgment.
• Friendly, patient, and respectful with a sincere interest in caring for others.
• Ability to lift up to 50 lbs and perform physical caregiving tasks.
• Maintain confidentiality and comply with HIPAA regulations.

Responsibilities

• Assisting with personal care: bathing, toileting, grooming, dressing, feeding, oral hygiene, nail care, shaving.
• Mobility support: ambulation, transferring, positioning clients in bed, using assistive devices.
• Supporting physical therapy plans, passive and active exercises under supervision.
• Monitoring and communicating changes in physical or emotional health.
• Performing light housekeeping: laundry, tidying bedrooms, kitchens, and bathrooms.
• Preparing meals (including special diets), shopping for groceries, and household supplies.
• Maintaining a clean, safe, and comfortable living environment.
• Using PPE when appropriate.
• Documenting care and submitting timely reports via telephony system.
