You will follow care plans, assist with daily living activities, monitor vitals, document care in the EMR, and collaborate with nurses and care teams to deliver high-quality, person-centered support.

A state HHA certification, CPR, and reliable transportation are required.

Qualifications

• Meets state and federal Home Health Aide certification requirements.
• Must be at least 18 years old.
• CPR certification required.
• Valid driver’s license and reliable transportation.
• Ability to read care plans and document services.

Responsibilities

• Assist patients with daily living activities including bathing, dressing, grooming, and toileting.
• Support mobility, transfers, and basic exercises per care plan.
• Maintain a clean, safe home environment.
• Prepare meals and assist with feeding as needed.
• Take and record vital signs when required.
• Document care in the EMR accurately and timely.
