Job summary

VitalCaring is seeking a Certified Nursing Aide (CNA) / Home Health Aide (HHA) to deliver compassionate, in-home care for patients in Houma, Louisiana. This PRN role focuses on personal care, safety, and patient dignity, supporting the care team with accurate documentation in the EMR system.

The ideal candidate will have a current HHA certification, CPR (where required), reliable transportation, and the ability to work independently while following detailed care plans.

Qualifications

• Meets state and federal requirements for Home Health Aide certification or training.
• Ability to read care plans and document services accurately.
• Excellent bedside manner and compassionate communication.
• CPR certification may be required per state guidelines.

Responsibilities

• Provide hands-on home health care with emphasis on safety and comfort.
• Assist with activities of daily living (bathing, dressing, grooming, toileting).
• Support mobility, transfers and exercises as outlined in the care plan.
• Prepare meals and assist with feeding as needed.
• Record vital signs and report changes to the clinical team.
• Maintain a clean, safe home environment and proper documentation in the EMR.
• Participate in team meetings and care coordination activities.
