Job summary

Grace Home Health in Lafayette, LA is seeking a Home Health Aide (HHA) to provide in-home personal healthcare under the supervision of a registered nurse. You will assist with activities of daily living, monitor patients, document care timely, and maintain a clean, safe home environment while traveling to assignments.

Candidates should have a high school diploma (preferred), CNA/HHA certification or recent completion, CPR and Hepatitis profile, and a valid driver’s license with insured vehicle;

Qualifications

• High school diploma preferred.
• 1+ year of home health care or CNA/HHA certification.
• English reading and writing ability.
• Proof of CPR and Hepatitis profile.
• Licensed driver with insured vehicle.

Responsibilities

• Provide direct patient care under direction of the RN and according to the Aide Plan of Care.
• Complete appropriate documentation in a timely manner.
• Practice accepted infection control principles.
• Provide a clean, safe, and comfortable environment which promotes respect for patient, privacy and property.
• Provide skills necessary to assist the patient with proper nutrition and adequate fluid intake.
