Job summary

Addus HomeCare in Jacksonville, FL is seeking a Home Health Aide to provide basic care to clients in their homes per a written care plan established by the Administrator.

The role includes recording care information, reminding about medications, meal preparation, assisting with therapy exercises, personal care, mobility, housekeeping, companionship, vital signs, and reporting changes to supervisors. Training, confidentiality, and compliance with Medicare/Medicaid/HIPAA are required.

Qualifications

• Minimum one year of personal care experience with three verifiable work references.
• Must be at least 18 years old.
• Completion of high school or GED certification.
• Six months training as a Certified Nurse Aide and competency evaluation.
• Must exhibit mature, responsible behavior and understand patient confidentiality.
• Must be able to read, write and speak English.
• Must have reliable transportation to and from assignments.
• Must maintain current credentials including CPR, First Aid, TB Test, Physical, Fingerprint Clearance Card.

Responsibilities

• Record pertinent information about the client's care.
• Remind clients to take medication at the appropriate time.
• Plan and prepare meals according to specific dietary requirements, including feeding if necessary.
• Assist the client with therapy exercises under the direction of a Registered Therapist.
• Assist with personal care including hair care, oral hygiene, bathing, grooming, and dressing.
• Assist with ambulation with or without mechanical aids.
• Assist with routine bodily functions such as toileting.
• Maintain a safe, clean environment through light housekeeping.
• Provide companionship and accompany the client to appointments.
• Turn or reposition bedridden patients.
• Examine patient for bruises or other injuries/wounds.
• Take vital signs and record input/output.
• Observe and report changes to the Agency Director or Service Coordinator.
• Prepare daily records and submit on date due.
• Participate in in-service education programs.
• Maintain confidentiality at all times; follow regulations.
• Maintain regular attendance and be flexible to agency needs.
• Follow Medicare, Medicaid, and HIPAA regulations and requirements.
• Perform other duties as assigned.
