Job summary

Care New England is seeking a Certified Nursing Assistant to work under the supervision of the Team Manager, collaborating with identified case managers to provide direct personal care to patients in accordance with the CNA plan of care. The role involves routine patient visits, assistance with daily living activities, adherence to care plans, and timely documentation of patient progress.

English proficiency and CNA certification renewal are required, with prior CNA/home care experience

Qualifications

• High school diploma or equivalent required.
• Must maintain CNA certification with continuing education.
• Minimum one year of CNA experience preferred; home care experience preferred.

Responsibilities

• Follows the established plan of care under supervision of the Case Manager to provide para-professional patient care.
• Makes scheduled patient visits at assigned times and coordinates with patient/family.
• Bathes, dresses, and assists with personal hygiene to enhance comfort and wellbeing.
• Assists with laundry, household chores, bed making, meal preparation, and activities of daily living.
• Assists with elimination needs of patients and monitors intake/output as directed.
• Documents care provided and reports changes to the Case Manager promptly.
• Reminds patients to take medications as directed by the primary professional.
• Reports unusual patient conditions and any potential abuse concerns immediately to the primary professional.
• Records vital signs under supervision and maintains daily progress notes as required.
• Prioritizes tasks and maintains daily schedules and documentation per agency policies.
• Answers general patient questions with courtesy and refers complex questions to appropriate personnel.
• Assists with physical and occupational therapy exercises as assigned.
• Participates in agency quality improvement activities and committees.
