Job summary

Care New England in Rhode Island seeks a Certified Nursing Assistant to work under the Team Manager and collaborate with case managers to provide direct personal care to patients, following the CNA plan to support progress toward outcomes.

Qualifications

• High school diploma or equivalent is required.
• Must maintain CNA certification through continuing education as applicable.
• Must be able to speak, read, and write English with proficient oral and written communication.

Responsibilities

• Follow established plan of care under supervision to provide para professional patient care.
• Perform scheduled patient visits and coordinate with Case Manager for care planning.
• Assist with hygiene, daily living activities, laundry, meals, and mobility as per care plan.
• Record intake/output, vital signs, and progress notes daily.
• Remind and assist patients with medications as directed by supervising professionals.
• Report changes in patient condition promptly to Case Manager.
