Job summary

Reserve Care in Ohio is seeking a State Tested Nursing Assistant to join our home health team. Provide personal care under nurse supervision according to the patient’s plan of care and collaborate with a transdisciplinary team.

You will perform ADL assistance, monitor patient status, document observations, and participate in care conferences. Flexibility to meet patient/family needs, including weekend shifts, is preferred.

Qualifications

• Must be able to read and write English proficiently.
• Must be state tested nursing assistant in good standing.
• Equivalent education/experience may substitute for minimum qualifications.

Responsibilities

• Provide personal care and assist with ADLs.
• Create a safe environment for patients and assist with transfers.
• Assist with passive and active ROM exercises as directed by the nurse.
• Prepare nutritional snacks as appropriate for the patient.
• Under nurse supervision, assist with pre-poured medications (Home Care only).
• Perform simple wound care, skin care, and bowel care protocols as directed.
• Record activities and observations in the clinical record.
• Complete charting, timesheets and mileage logs as defined by policy.
• Attend and contribute to patient care conferences and meetings.
• Participate in on-call rotation (Home Care only).
• Assist Team Leader with scheduling, supply organization, and office duties.
• Provide basic caregiver education to family members.
