Job summary

SKY HEART HOME CARE SERVICES is seeking a PCA/CNA to join our home care team. You will work directly with clients in their homes under the supervision of the Service Supervisor, providing personal care and assisting with activities of daily living.

The role emphasizes compassionate care and documenting client progress to support overall wellbeing. Must be at least 18, have a high school diploma, and preferably one year in healthcare.

Qualifications

• Must be at least eighteen (18) years of age and have a minimum of a High School Diploma.
• Must have at least one year of experience in the healthcare profession preferred.
• Must be in good standing with the health care registry.
• Must have minimum two (2) satisfactory references from previous employers.
• Current certificate in first aid and CPR preferred.
• Valid driver’s license, reliable automobile, and current auto insurance required.

Responsibilities

• Assists client in all activities of daily living/hygiene assistance, bathing, grooming, and linen changes as outlined in the POC.
• Prepares nutritious meals within the patient’s diet and assists client with eating when necessary.
• Performs light housekeeping chores to facilitate client’s self-care.
• Notifies supervisor of any acute or pertinent changes in client’s condition.
• Maintains confidentiality regarding the client’s condition and family.
