Job summary

Qualifications

• Current certification as a Home Health Aide (HHA) or Certified Nursing Assistant (CNA).
• High school diploma or equivalent; some college or healthcare training is a plus.

Responsibilities

• Provide high-quality, patient-centered care to clients in their homes, focusing on physical, emotional, and social well-being.
• Assist with daily living activities, such as bathing, dressing, grooming, light housekeeping and meal preparation.
• Monitor and report changes in clients' condition, ensuring prompt communication with healthcare providers and family members.
• Collaborate with a multidisciplinary team to develop and implement personalized care plans.
• Maintain accurate and detailed records of client care and services provided.
