Qualifications

• Experience in home care, community care, assisted living, nursing homes, or hospice.
• Knowledge of vital signs monitoring and patient observation.
• Ability to assist with ADLs with compassionate patient care.
• Familiarity with medication reminders and care planning processes.
• Experience with dementia and mobility challenges.
• Strong communication with clients, families, and healthcare teams.
• CPR and First Aid certification preferred; training provided if needed.
• Knowledge of HIPAA regulations for confidentiality.

Responsibilities

• Assist clients with ADLs including bathing, dressing, grooming, and mobility support.
• Monitor and record vital signs (BP, pulse, temperature).
• Provide medication reminders following care plans and HIPAA guidelines.
• Prepare meals and offer nutritional support tailored to needs.
• Support memory care and cognitive impairments.
• Use assistive devices to ensure safe transfers and mobility.
• Observe and report changes to the care team.
• Perform light housekeeping and document services accurately.
• Support patients during social activities and promote engagement.
• Follow safety protocols including CPR, first aid, and infection control.
