PossAbility, LLC in Illinois seeks a Home Care Aide to support individuals with IDD and TBIs to live independently and safely at home. You will assist with daily tasks, provide emotional support, and advocate for client goals. You will work as part of a person-centered team, coordinating with supervisors, documenting care, and ensuring compliance with HIPAA and agency policies. A compassionate, patient-first attitude is essential.

Qualifications

• Experience in Direct Care or home care settings preferred.
• Knowledge of personal care tasks, household management, and basic meals.
• Ability to work with individuals with IDD and TBIs, and to advocate for clients.

Responsibilities

• Provide hands-on assistance with dressing, bathing, grooming, toileting and ADLs/IADLs.
• Adhere to HIPAA, agency policies, and mandated reporting requirements.
• Communicate with supervisors, clients, and family/natural supports; document care and progress.
• Engage in ongoing training and demonstrate flexibility to meet client needs.
