Job summary

hce-careers is seeking a compassionate Home Care Aide to work with patients in their homes, following a one-on-one care plan. You will provide personal care, support daily activities, and help maintain a healthy home environment.

Ideal candidates are detail-oriented, organized, and possess strong communication skills. Prior Home Health or Mental Health experience is a plus; schedule includes weekday hours with occasional weekends and per-diem opportunities.

Responsibilities

• Coverage is needed Monday through Friday (9am to 5:00pm) (11pm to 7am) and every other weekend Per-Diem (As Needed)
• Assist the client to and from doctor’s appointments
• Maintain records of patient care, conditions, progress or problems to report and discuss observations with supervisor
• Engage patients in exercises or other activities
• Give medication reminders
• Assist patients by doing laundry, cleaning the home, or assisting with personal care (if needed) and cooking
