Job summary

Cornerstone Support Services is seeking a Home-Based Case Worker to connect families with resources and support to meet their goals. The role involves assessing needs, coordinating services, and ensuring safety in supervised visits in the community or home.

You will document sessions within 48 hours, maintain client confidentiality, and collaborate with a supervisor to address safety concerns and create action plans. A flexible schedule and valid driver's license are required.

Qualifications

• Requires high school diploma or GED and 4+ years direct service experience.
• Bachelor’s or Master’s degree in a related human services field preferred.
• Must have CPR, TB, and First Aid certifications and ongoing training.

Responsibilities

• Create a consistent weekly schedule for parent and child.
• Transport child(ren) to visits in community or home when permitted.
• Conduct supervision intake and ensure safety and confidentiality.
• Document visits and monthly reports within required timeframes.
• Develop safety, treatment, and crisis response plans as needed.
• Maintain HIPAA compliant documentation and case management records.
