Qualifications

• Experience with Activities of Daily Living (ADLs) and Patient Care in a home or community-based setting.
• Strong Communication skills for coordinating schedules, collaborating with caregivers, and interacting with clients and families.

Responsibilities

• Assist clients with activities of daily living, including personal care, basic mobility support, light homemaking tasks, and companionship.
• Schedule caregivers for client visits, coordinate shifts, update schedules in the system, and communicate changes to staff and families.
• Monitor client needs, report changes in condition to supervisors, and ensure timely, reliable care coverage.
