Job summary

PACS is seeking a dedicated caregiver to provide routine daily care to residents in our facility, following each resident's assessment and care plan. You will greet residents, assist with daily living activities, and ensure a warm, safe environment.

This role requires a high school diploma or equivalent, good communication, and the ability to report changes to nursing staff. You will work in shifts from 2pm to 10pm or 10pm to 6am, support with meals, mobility, and personal care while maintaining

Qualifications

• Must possess, as a minimum, a high school education or its equivalent.
• Ability to read, analyze, and interpret general resident care guidelines and policies.
• Ability to write clear notes and reports on resident care and daily activities.

Responsibilities

• Provide routine daily care to residents per the resident's assessment and care plan.
• Report changes in residents’ condition to Health and Wellness Director.
• Greet residents, escort them to rooms, and maintain a warm, safe environment.
• Assist with daily living activities including meals, mobility, and personal care.
• Maintain confidentiality of resident information and follow privacy policies.
